Our Counselling department is current seeking a:

Case Manager / Disability Supports Specialist, VANCOUVER BC

Full-time, Contract (1 Year) Position

Start date: September 17, 2018

Job Summary:

The position supports individuals to achieve ongoing labour market attachment. Reporting to the Head of Counselling Services, the Disability Supports Specialist will serve Deaf and Hard of Hearing clients whether they are employed, unemployed, or underemployed.  The new Case Manager position is responsible for processing all disability support applications for Hearing aids, Assistive Listening Device Systems, Sign Language interpreting requests and other assistive technologies through the Employment Program of British Columbia (EPBC).

Duties Include:

  • Conduct various needs assessments with clients
  • Complete paperwork in accordance with EPBC policy and guidelines
  • Meet key performance measurements as outlined by contractors/stakeholders/WIDHH
  • Liaise with internal and external professionals: organizations, agencies, and government
  • Complete comprehensive evidenced based reports
  • Provide and monitor ongoing client case management activities through database tracking
  • Provide ongoing documentation of client progress notes in the Integrated Case Management System
  • Achieve monthly, annual goals and objectives
  • Maintain high standards of service quality
  • Provide public presentations and community outreach
  • Provide referrals to outside agencies

Qualifications: 

  • Ability to establish and maintain rapport with clients using a client-centered approach
  • Knowledge of technology, communication devices, disability supports and accommodation strategies
  • Bachelor’s Degree in relevant, disability accommodation , social services, community, or an equivalent combination of education, training, and experience
  • Solid knowledge of MS Office, MS Word, MS Excel, PowerPoint, Outlook
  • Knowledge and experience of the Integrated Case Management System (not required but is considered an asset)
  • Aptitude for navigating complex database systems
  • Attention to detail, high accuracy and adaptable
  • Highly organized, efficient, and reliable
  • Excellent communication and written English skills
  • Ability to organize and prioritize, excellent time management skills
  • Good understanding of Deaf and Hard of Hearing accommodation and communication needs
  • American Sign Language is an asset for this position
  • Demonstrated sensitivity to working with a culturally diverse caseload
  • Effective public presentation skills
  • Team player, highly motivated and focused on achieving success
