Supported Independent Living Facilitator

Work collaboratively with the department Director or Program Manager to oversee, implement, and maintain the program in a fun and flexible environment.

Coordinate and manage a case load of assigned responsibility. Act as a resource to individuals served, their respective family members or representatives and the care providers/support staff. Perform a variety of professional activities in support of the Program including but not limited to administration, home visits, contract enforcement, inspections, placements, advocacy, special projects and program development.

Regular schedule: Monday to Friday 8:30am to 5pm (unpaid lunch) 40 hours per week

• Must be able to travel throughout lower mainland and Fraser Valley
• Position includes paid rotating on-call duties

Job Qualifications Include:

• Minimum Bachelor of Social Work or other Social Science
• or Social Services Diploma plus experience in the social services field
• Minimum of One (1) year of case management experience
• Minimum of One (1) year experience with individuals with disabilities
• Fluent in ASL

Job Requirements Include:

• Criminal record check
• TB test
• First Aid and CPR
• Minimum of three (3) contactable references
• Valid BCDL and Reliable vehicle to use for work purposes

Additional Requirements:

• Experience with advocacy
• Experience in the public or non-profit sector
• Experience working with individuals with physical, developmental disabilities, brain injuries, and/or complex health issues
• Experience in the social services field
• Experience working in a team environment with little supervision
• Experience working with a case management team
• Demonstrated ability to deal with crisis and emergency situations
• Computer literacy and experience with databases
• Excellent written and verbal communication skills
• Excellent time management skills
• Knowledge of community resources

Start Date: ASAP

Compensation starting at $20.38/hr and full benefits upon meeting eligibility requirements after three months.
